Board Self Assessment Questionnaire

The questionnaire below is designed to help the Board to assess how well it is functioning and to identify areas where the Board as a whole might improve. It may also be a helpful indication for board new members of the level and nature of engagement required of active Board members.

Download the questionnaire here.

Read each responsibility carefully and tick the most relevant response.

The assessment facilitator will endeavour to obtain a completed questionnaire from every Board member, analyse the results and present a consolidated report in which individual responses are guaranteed anonymity.

Scale and Open Ended questions
The purpose of the questions is not to get a scientific survey of the board, but to stimulate discussion among the members at the time of review

1. Did the work of the board meet your expectations?
Yes 1. 2. 3. 4. 5 No

Why?

Why not?

2. Did you feel included?
Yes 1. 2. 3. 4. 5 No

Why?

3. What would have made your experience a better one?

4. What do you think is important for the board to focus on next year?.

Emphasis on the future:

  • What will you do differently to improve next time?
  • What one thing done differently would have most impact?
  • If you made those changes, what difference do you think it would make?
